Elbow ossification centers – CRITOE
Why Is This Important?
- Pediatric elbow X-rays are notoriously confusing
- Ossification centers can:
- Mimic fractures
- Hide fractures
Therefore, you must know:
- Order of appearance
- Age of appearance
- Which centers should be present at a given age
Mnemonic: CRITOE
| Letter | Structure | Age (Years) |
|---|---|---|
| C | Capitellum | 1 |
| R | Radial head | 3 |
| I | Internal (Medial) epicondyle | 5 |
| T | Trochlea | 7 |
| O | Olecranon | 9 |
| E | External (Lateral) epicondyle | 11 |
Memory Trick
1 – 3 – 5 – 7 – 9 – 11
Key Clinical Concepts
1. Age Correlation is Critical
- Always interpret X-ray based on child’s age
Examples
- Age 3 — Expect:
- Capitellum + Radial head
- Age 5 — Expect:
- Capitellum + Radial head + Medial epicondyle
- Age 11 — Expect:
- All centers
2. Gender Variation
- Girls:
- Ossification appears ~2 years earlier
– So:
- Use age as a guide, not an absolute rule
Clinical Application
Scenario: Missing Ossification Center
- You see:
- Trochlea (appears at 7 years)
- But:
- Medial epicondyle (should appear at 5 years) is missing
-Conclusion
- It should be present
- Suspect:
- Displaced fracture
- Possibly intra-articular entrapment
Classic Clinical Case
-Medial epicondyle fracture + elbow dislocation
- Fragment may be:
- Trapped inside the joint
Clue
- “Missing” medial epicondyle on X-ray
Key Diagnostic Rules
Rule 1
– If an earlier center is missing:
- Suspect injury
Rule 2
– If a “center” appears earlier than expected age:
- It is likely a fracture fragment
Example
- Olecranon appears at 9 years
- Seen at age 5 — Not normal — think fracture
Rule 3
Ossification Centers
- Smooth
- Rounded
- Symmetrical
Fractures
- Irregular
- Sharp edges
- Displaced
Common Pitfalls
- Mistaking normal centers for fractures
- Missing incarcerated medial epicondyle
- Ignoring age mismatch
- Forgetting earlier ossification in girls
Exam Pearls (High-Yield)
- CRITOE = order + timing
- Most clinically important center:
- Medial epicondyle
Golden Rule
“Missing medial epicondyle = look inside the joint”
Always Correlate
- Age
- X-ray findings
- Clinical scenario
Final Takeaway
- Pediatric elbow interpretation is pattern recognition + age correlation
- Mastering CRITOE helps you:
- Avoid missing fractures
- Avoid overcalling normal anatomy
