Big Toe Pain: Causes, Diagnosis, and Management
Overview
Pain in the big toe (hallux) is a common clinical complaint, particularly due to its role in:
- Weight-bearing
- Gait
- Push-off during walking and running
The first metatarsophalangeal (MTP) joint is subjected to repetitive stress, making it prone to multiple pathologies
Common Causes of Big Toe Pain
- First MTP joint arthritis
- Hallux rigidus
- Hallux valgus (bunion deformity)
- Gout
- Turf toe
- Sesamoiditis
First Metatarsophalangeal (MTP) Joint Arthritis
Definition
Degenerative arthritis affecting the first MTP joint.
Clinical Features
- Pain in the big toe joint
- Stiffness
- Swelling
- Reduced range of motion
Advanced Disease
- Severe arthritis may require:
- First MTP joint arthrodesis (fusion)
Hallux Rigidus
Definition
A form of degenerative arthritis characterized by:
- Stiffness
- Limited dorsiflexion of the big toe
Pathology
- Dorsal osteophytes on the metatarsal head
- Partial preservation of joint cartilage
Clinical Features
- Pain during dorsiflexion
- Swelling
- Restricted motion
Treatment
Conservative
- Shoe modifications
- Orthotic inserts
Surgical
Cheilectomy
- Removal of ~25% of dorsal metatarsal head
- Excision of dorsal osteophytes
Contraindication for Cheilectomy
- Pain throughout mid-range of motion
Suggests advanced arthritis ? requires fusion instead
Hallux Valgus (Bunion Deformity)
Definition
Deformity characterized by:
- Lateral deviation of the great toe
- Prominent medial eminence of first metatarsal head
Symptoms
- Pain over medial MTP joint
- Shoe irritation
- Difficulty with footwear
Epidemiology
- More common in women
Treatment
Conservative
- Wide footwear
- Orthotics
Surgical
- Indicated when:
- Pain is significant
- Shoe wear becomes difficult
Gout
Definition
Crystal-induced inflammatory arthritis due to monosodium urate deposition
Common Site
- First MTP joint (most commonly involved joint)
Pathophysiology
- Needle-shaped crystals
- Negatively birefringent under polarized microscopy
Clinical Features
- Sudden onset severe pain
- Swelling
- Redness
- Warmth
Diagnosis
Gold Standard
- Joint aspiration:
- Needle-shaped urate crystals
Imaging
- X-ray:
- Punched-out erosions
Important Note
- Hyperuricemia does not always lead to gout
Turf Toe
Definition
Injury to:
- Plantar plate
- Sesamoid complex of first MTP joint
Mechanism
- Hyperextension injury
Common In
- Athletes
- Artificial turf sports
Clinical Features
- Pain at base of big toe
- Swelling
- Difficulty with push-off
Imaging
X-ray
- May show sesamoid injury
MRI
- Demonstrates plantar plate injury
Sesamoiditis
Anatomy
Two sesamoid bones beneath first metatarsal head:
- Tibial sesamoid
- Fibular sesamoid
Function
- Act as pulleys for flexor tendons
- Reduce friction
- Absorb weight-bearing stress
- Protect flexor hallucis longus tendon
Clinical Features
- Pain at plantar aspect of big toe
- Pain aggravated by weight-bearing
Treatment
Conservative
- Activity modification
- Orthotics
- Padding
Surgical
- Considered if symptoms persist
Diagnostic Approach
Key Components
- Detailed history
- Focused clinical examination
- Imaging when required
- Joint aspiration (if gout suspected)
Key Clinical Points
- Most big toe pain arises from first MTP joint pathology
- Overlapping symptoms require careful differentiation
- Early diagnosis helps prevent progression and disability
Take-Home Message
- The first MTP joint plays a critical role in gait and is prone to multiple conditions
- Causes range from:
- Degenerative
- Inflammatory
- Traumatic
- Management should be individualized based on diagnosis and severity
