This is a severe unstable slip
Need to work up for endocrinological and metabolic causes
if it is redused even with a gentle closed reduction and fixed there are high chances of AVN 30 to 50 % This is primarily because of kinking of MCF vessel by posterior callus
If oinned in situ AVN can be avoided but it leads to femoro acetabular impingement and a deformity which will not remodel
My choice is to do safe surgical dislocation of hip using Ganz approach do modified dunn procedure to correct femoral hrad alignment by shortening and excision of callus and fix with a cancellous screw
